When does that error happen ? When installing ? If so, it's unrelated to this extension, it looks more like a setup that's not compatible with composer.

and install failed

Could we maybe get this split out of the extension discussion ? It doesn't seem related to the extension itself.

I believe you're using flarum-cv ? Do other extensions install correctly (in particular flagrow ones) ?

Not sure where shell_exec is called, but it's probable it's a Composer requirement. Check Composer documentation for how to install/configure composer.
