[deleted] there is some padding-left on .DiscussionListItem-content you will need to change if you want the discussion to take the space of the avatar.
To add the discussion author beside the last reply, you'll need an extension to add the text. I don't think any extension does that at the moment. Let us know if you're looking for help developing such an extension, including what you've tried so far.
