Please don't double post, let's stick to this discussion. Looks like the permissions are wrong, somehow you have a duplicate entry in group_permission. You can try truncating that table and then - once the upgrade succeeded - update the permissions again in the admin.

Do you have phpmyadmin? If so you should be able to truncate that table from there..

No truncate. Truncating empties the table and resets the auto increment. Dropping a table deletes the whole table.

sazib the TRUNCATE option should be found under Operations in phpMyAdmin.

Do not DROP. That will remove the table completely. TRUNCATE just deletes the content of the table.

sazib my recommendation would be the following:

In case you have no posts/discussions/users ; create a new forum, if the issue occurs again after a clean installation report back here.
In case this is an existing, active forum I recommend getting assistance as you've shown no clear ability to resolve this yourself. Your use case is exactly why Flarum is in beta and states "do not use in production unless you can get your hands dirty to fix some issues".
Last option would be to clear (truncate) all tables except those that hold important information like posts, discussions, users and discussion_tag; to know which ones would require information on all the tables in the database, therefor see previous item.
