none it seems like the extension is disabled and you're affected by that odd rendering bug that hides the title and checkbox. Are you using Composer v2 ? If so, try downgrading to Composer v1, delete vendor folder and composer.lock file, and run composer install again.
If you are not using Composer v2, you could try resizing your browser to see if the name of the extension and the checkbox show up. If it still doesn't show up, can you share what browser and version you are using, and the output of your php flarum info ?
