Primary Tags Missing on Permissions Page

I'm getting some odd and random issues under the permissions where only the top level primary tag is the only one that displays for me to set permissions on. Any other primary tag nested/second level hierarchy is missing.

This issue has been persistent since it was a new install as soon as I nest primary tags under each other.

Again it's very random when it happens. Any suggestions on where to start checking?

Jeff-B you mean the tags are sometimes listed and sometimes not? Does it happen even if you don't enable/disable any extension in the meantime?

Did you try using different browsers or admin accounts?

Nested tags sometimes show and sometimes don't. Enable/disable of extensions have no effect.

Browsers tried so far: Firefox Dev, Chrome. Both have same issue.

I think I found the issue.

Looks like the Asirem theme is causing it possibly because of the styling in the Admin area. When I disable it, the tags reappear and then disappear when I enable it again.
