Joro I encountered that in "All discussions" the last person who replied is actually not shown under the discussion, but the one who replied before that. We are using the last version of Flarum.

Joro What we've done is we've simply translated the flarum-ext-english yml files with Bulgarian language.
jordanjay29 That's partly what I meant about custom edits. Any other modifications to extensions that you've done? I can't see any reason why, with the extensions you have, this would be an issue. By "latest version" do you mean Beta 5? Or a dev-master version?
Joro jordanjay29 The issue is definitely the Approval extension, but I don't know what. After trying to deactivate and activate Flags and Approval, now I have another bug which came up, with the Flags. If both Approval and Flags are active it is shown that I have flagged task but when I click on the flag button nothing shows, just the loader. Subsequently if there is an approval needed the flag button shows 2 flagged tasks but yet again on click nothing is shown. Is there a way to reinstall both Approval and Flags extensions?
Joro Beta 5. I just tried to deactivate Approval (v0.1.0-beta.5) and posted a new comment, which showed in All discussions as the last one. So I guess there is an issue with Approval extension.
jordanjay29 Joro Sure, SSH into your server to the flarum root. composer remove flarum/flarum-ext-approval and composer remove flarum/flarum-ext-flags Then composer require flarum/flarum-ext-approval and composer require flarum/flarum-ext-flags I'd recommend disabling the extensions before removing them, because Flarum doesn't dump extension cache until you click disable/enable.
