This extension wasn't worked on since 2016 beta 5. I can imagine it's broken. To be sure:
- run
php flarum cache:clear
- clear cloudflare cache in case you have that
- clear your browser cache
If this doesn't work, use composer to uninstall the extension and comment on the discussion here so we can initiate the process of marking it as abandoned.
