askvortsov well, you could definitely use Formulaire to build a survey-style form that users can fill in. Not all field types visible in the sample quiz are available in Formulaire right now but it's planned to add them (multi-choice, ...)
Defining good answers and having a results page would be a nice feature. Not sure whether I'd prefer to have that built-in or as a companion extension. I was not planning to work on such a feature but if there's interest in sponsoring this additional feature on top of the existing extension this could be done.
I'd estimate building something similar to the linked sample quiz as a Formulaire companion extension would probably be 5 to 10 hours of work at my open-source rating which also applies to sponsored features in my premium extensions.
Building something like that from scratch including a form builder is probably multiple tens of hours.
