Happens on my side too, also when I receive a mail about mention, it'll say that apparently "CW_Kevo mentioned you [...]" (I mentioned me, but I didn't).
First one seems like a cache issue related with recent Discuss beta.15 update to me, second one is probably a bug with placeholders in the notification
