Create a custom extension

Hello everyone,
I am looking to get a quote for a custom extension.

The extension should work something like this:

after email confirmation users must be forced to make a multi selection from a dropdown menu
admin should be able to create the selection name/model with image, category and subcategory
the selections should be displayed on profile page and signature, under user comment

I was able to do this a bit with Masquerade but there is no options to create a category, to add an image and to show it under comment.
