This happened to me multiple times before as well, I ended up reverting to a backup each time. I'll have to try this solution next time I update.

Circa I ran into this with another instance and used Ben's queries to fix it. Make sure to delete the given table if it mentions "table already exists" after running the alter table queries.

xiahuan710 drop the table in your database: post_mentions_tag using a database manager or run the query there (drop table post_mentions_tag;) then try again from the commandline using php flarum migrate.
