Appears to be functioning now.

A few things I noticed:

The style of the category (primary tag) title is practically illegible with the default Flarum styling, a lighter color would be useful
The extension appears to treat Primary Tags as categories, and Secondary as forums (might need a short user's guide to explain this)
No discussions tagged only with Primary Tags show up, nor any statistics of the Primary Tag's discussions
Koseki's page ignores non-nested Secondary Tags completely

EDIT: It seems to be working well for me and it does the job really well. I do not have much to add to jordanjay29's excellent summary. Here are a few more suggestions:

Have more than one theme. I am fonder of two column designs since I find three columns a bit busy. I prefer a style more like this, though there are certainly many great ways to make a two column forum theme:
